Transition to Adulthood Resources

FACT Oregon empowers families, youth, and communities to navigate disability systems, advocating together for disability justice and policy change so that people with disabilities have what they need to thrive at home, in school, and in their communities. FACT Oregon works toward a future in which communities are accessible and inclusive and where the self-determination of people with disabilities is assured.

Peer-delivered services are free for families and include 1:1 support, trainings, resources, parent leadership development, and advocacy. We are Oregon’s statewide Parent Training and Information Center (PTI) for Special Education and a member of the Oregon Consortium of Family Networks, which serves people with intellectual and developmental disabilities and their families.

The Independent Living Program (ILP) helps youth who are, or were, in foster care to become self-sufficient adults. It gives an opportunity to learn valuable skills necessary to make a successful transition from state or tribal custody to living on your own in the community.

The purpose of the YTP is to prepare students with disabilities for employment or career related postsecondary education or training through the provision of a comprehensive array of pre-employment transition activities and supports.

Youth ERA works to empower young people and create breakthroughs with the dedicated systems that serve them.
