1st Choice Assisted Care, LLC is a private Certified Medicaid Agency that is the employer on record for all of our employees. We hire, train, match, support, and supervise all of our Providers who serve clients under the K-Plan. We also work with local CDDP/Brokerage offices and Vocational Rehab offices to support clients: as clients become eligible, their Service Coordinator or Personal Agent may refer them to our agency. 1st Choice works directly with clients and their support systems to match them with a qualified provider- we do our best to successfully and a sustainable match clients with providers, based on the individual needs of each client.
